Medication Therapy Management Interventions in Outpatient Settings describes intervention components and implementation features for outpatient medication therapy management (MTM) interventions with comprehensive medication review, follow-up, education, and care coordination. Assesses the effectiveness of these MTM interventions on intermediate, patient-centered, or resource utilization outcomes; identifies intervention features and patient characteristics that moderate the effect of an intervention on outcomes; and assess harms associated with interventions.

However, there is variability in how this process is taught and practiced. To promote consistency across the profession, national pharmacy associations used a consensus-based approach to articulate the patient care process for pharmacists to use as a framework for delivering patient care in any practice setting.
