Innovative Approaches to Improve Transitions of Care: An update from the Hawaii Pharm2Pharm Initiative

The Hawaii Hospital Consulting Pharmacist to Community Consulting Pharmacist care transition and care coordination, or “Pharm2Pharm,” model was designed to leverage the expertise of pharmacists to close gaps in care for those patients at high risk of medication-related hospitalization and emergency room visits. This innovative transition of care program has been launched in three rural counties in Hawaii and urban Honolulu county. As of May 2014, over 1,200 patients have been enrolled by hospital consulting pharmacists and handed off to community consulting pharmacists after charge.
Join representatives from the “Pharm2Pharm” model and the Hawaii HIE to learn about the success of the program and HIT functionality leveraged through the HIE.
